Test connection failed to get form ID from microsoft forms on power automate

I have created a survey form and trying to get a connection flow to power automate but it is giving me an error saying the test connection failed and I am unable to get the form into power automate.

How do I solve this issue?

According to the information you provided we have no way to determine the specific cause of the problem, can you provide more information? For example: what is your data source? How did you create the connection? Can you provide a screenshot of the error report?

I have created a survey form using microsoft forms. I have created a new dataset using push API. Later I created a report. 

On Power Automate, I am trying to add flow using automated cloud flow. At this point I am getting an error saying that the test connection is failing. I am trying to add rows to my dataset using this flow. Because of the error I am unable to.

I am attaching a screenshot of the error with this. Please find below and help me with the issue.
