Relationship Error - Data not Aggregating

Hello! 

I currently am testing excel results from an MS Forms Excel (in future I am looking to pull directly from MS forms once I am ready to activate my survey).

Here is my current relationship model -

My primary key is Department ID, Foreign Keys are Dates and Performance ID
I would like to relate my Department Key to my Survey Responses table on Department Name. 
But keep recieving an "ambiguous path" error message. I have removed all relationships and relinked them. 

As my dashboard does not reflect the filtered infromation for Survey results for May 14 (completion date) -

I would love to know what may be going wrong.

Hi  @powerbinovice01 ,

The primary key column (“Department ID”) should have unique values. If there are duplicates, it could cause issues with the relationships.

Hi Caitlyn,

The Department ID does have "unique identifiers" - 

But most importantly I am trying to build a direct relationship on "Department" column in the survey responses to Department Key on Department Name 

Where do you think the issue may be coming from? I analized the text and there are no additional spaces or incorrect spellings.
