I've had some really really weired things going on with the Service in the last couple of days. Something strange is going on.
One example - I published a report to a single workspace and it ended up appearing in ALL of my workspaces; thne, deleting one of the extra reports deleted it from all the workspaces again.

You mean even you rename your pbix file and publish a new report, you will see the same issue either? Could you please try these steps and see whether this issue still happens?
1 Clear the cookie/cache of your browser and try again
2 Try with other browsers
