We have less the 100 people who can access the app.  There have been no changes to anything that I can tell related to the app, but it will not update (cannot add/ remove people or add/ remove reports).  Haven't had any issues with it before.

Hi @blombergs5 ,

According to the error message you provided, the reason for your error was : There are multiple explorations referenced by report. Clean up the duplicate explorations and update the app again.

Check if you have duplicate reports, datasets or multiple references.

That fixed the issue, somehow we had copied a report and had 2 with the exact same name.
