When a text tile is added in a dashboard, and (by mistake) there is no title / subtitle / text set, then the "more options" button on the tile is not visible:

Based on my test in Chrome browser version 74.0.3729.131 (Official Build) (64-bit), after we add a text box tile via Add Tile feature in a dashboard, when we move the cursor on the tile, the "..." will display on the top-right corner of the tile. After we refresh the browser page, the tile still display in the dashboard.
In your scenario, please test again to see if the same issue occurs.

Update: after refreshing the dashboard page, the problematic tile has disappeared altogether.
