Allow data entry by users using Power Apps

In Power BI my report shows debt for every customer. I need to allow the account manager to ENTER their comments for the debt. Then I need them to "submit" the comment so I will KEEP the comments HISTORY.

I understand I can achieve this using PowerApps visual, but I need more step-by-step guide.

Especially - on how do I keep the history? I do have a Sharepoint I can use for this.

There are lot's of videos on PowerApps - can you please help me find the right tutorial.
