Adjusting my query to changes in Source file

Hello everyone, 

I need to extract data from a pdf table on a quarterly basis and this time Power Query reads out the heading in a Column 11 instead of Column 10.  Is there any chance to get the first two rows back to column 10? The issue is that the following rows are in correct order. I would be pleased to get some tips on how to handle this.

Demo file attached. Please try below steps:

1. Add a new [Index] column

2. Create new conditional columns based on [Index]

3. Remove incorrect columns and [Index]
