How to map change in status over a period of time

Hi,

 

I apologize if this is a noob question as I am brand new to Power BI. I want to be able to make a line graph that shows status changes for a work item over a span of 12 months with multiple lines for each year. 

 

For example, Item ABC will be in 'ready to start' status in january and will change to several different status's over the year eventually ending as 'completed'.
