Help Creating Control Chart From Simple Dataset

Hello - I am trying to create a control chart from a very basic data set (report_month and risk_score).  I've watched a few videos and read a few articles and I have a general idea how to do this but none of my research has landed on the right approach.  I'm hoping someone can lend some assistance.  Sample files can be found at the link below.

For my central line, I want this to represent the average of the first two years of my dataset (2022 and 2023)  The dataset goes from 2022 to 2025.
Upper and lower bounds should 3 standard deviations from the central line.

 

Also - as an added bonus I would like to create another control chart using the same dataset but this one would be a p-chart where the upper and lower limits move based on the population size.  That calculation is a bit more complicated.

@phcaptjim  The dataset provided and the requirement in the image is confusing. Can you please elaborate the requirement ? I can only see risk_score and month in the dataset you provided. How is defects and sample size is derived ?

Thanks @lbendlin for the tips.  I've attached a link that has a sample XLSX and PBIX file.

Upper and lower bounds should 3 standard deviations from the central line.

Your data is way too volatile for that.  The standard deviation is already 25.67

This process is not controllable that way.
