Hello - I am trying to create a control chart from a very basic data set (report_month and risk_score).  I've watched a few videos and read a few articles and I have a general idea how to do this but...

phcaptjim  The dataset provided and the requirement in the image is confusing. Can you please elaborate the requirement ? I can only see risk_score and month in the dataset you provided. How is defects and sample size is derived ?
