I created a rule that any score of ITR less than 75.7% is to have an icon next to it.
Both table A and table 5 have the same rules for the conditioning formatting but it doesn't work properly in table A.
Does anyone know what I'm doing wrong here? T

Does the [sample] column have sub-hierarchy?
It's hard to tell what is wrong only by the screenshots. Better to share the data model and the formula to us.

@LRCSG change the percent to number in conditional formatting and change 75.7 to .757 (decimal 757)

The problem is the 75.7 is a measure. changing the format to number doesn't solve this problem.
