Live connection and SQL Server

Is it possible to make a live connection with a SP or a Table on SQL Server? I have been reading about Streaming Dataset but I have not came accross and example of what I'm trying to do:

A live connection with a SP on SQL Server so that the data refreshes every time it changes on the table.

Just remember that there could be a big load on the SQL Server system when the queries are running?
