Cannot Visualize Data

Hey Everyone,

I'm unable to visualize data although all fields can seen on the right side of the screen. 

I have created a dataset and made a flow ( Microsoft Forms + Power Bi). 

I have reviewed every step I made and I'm sure they're correct, I'm new to power bi and wish if I could get some help about this weird problem. Feel free to ask any questions.
