We have a number of servers licensed for SQL Server Enterprise with SA and would like to run PBI Report Server. Does this PBIRS have to be installed on a server that is licensed or can we install the Report Server on a separate app server with the RS database on a SQL server that is covered by SQL Enterprise with SA? Is there any clear documentation on this? Thanks

You must install both (RS database on PBIRS app server) on servers covered by MSSQL EE w/SA licences ...
Generally there two options:
-You must have licence for every vCPU on each server where you running MSSQL DB or PBIRS (or also SSAS or SSIS).
-Or you must have licence for all cores on some physical server, and then you can run any number of virtual servers with any number of vCPUs with any combination of MSSQL, PBIRS and so on, which is on this physical servers.
