Yes, you can use an existing SQL Server. Or if you don't have an existing one that is suitable Power BI Report Server comes with a limited use license for SQL Server so you could install a new instance on your Report Server provided it is only used for the Report Server databases and does not handle any other workloads.
To use Report Server, you need to either have Power Bi Premium or SQL Server Enterprise Edition. And there is no additional licenses in Report Server itself. You can use all features with the above 2 licences.

There is no separate license, it's part of the Power BI Report Server licensing.
