help with visual showing only questions when answered

really hope you can help x

i have a data source which shows question answers for inspections. when of these questions is a fail, then it will ask a follow on question as to how bad the fail is.

what i want to show is a list of the failed questions, and then the "defect rating " attributed. the problem is that the follow on question which is the next column in the data has no relationship to the column previously and there can be multiple failed questions on the same row.

basically i want to show a table that shows all the failed questions, and then the defect rating for that question, "minor,major"

@fstupot , You can use a slicer of failed status/status and display rest in a table visual.

Can you share sample data and sample output in table format? Or a sample pbix after removing sensitive data.
