I need a chart which present the above table as below:

& the tooltip could present the recored number.

You could transposed table in power query. then use matrix.

1.Transposed Table

2.Promoted Headers

3.Split Column by Delimiter

4.use matrix.

5.set tooltip in page2

6. apply it.

The final output is shown below:
