Published report reverting to a previous version after several hours in both service and app

We're having repeated issues with a published report working correctly in both the service and the app and then mysteriously reverting back to a previous version. Specifically, an RLS filter is fully functional and then disappears. Our UAT version works perfectly; we save it as (overwrite) a differently named previous file (Active) and publish that Active file. The UAT file continues to work while the Active file does not.

Ask the tenant admin to give you the logs for the pbix upload, dataset update and app update activities.  Then you can find out who did it and when.
