I have a card that has a title and a value. I've created a custom tool tip which works perfectly over the value but there is still a tool tip on the title.

In Power BI, tooltips are applied to the entire card, including both the title and the value. Base on my research, there is no built-in feature to disable the tooltip for the card title while keeping it for the card values.
You can submit a new idea. It is a place for customers provide feedback about Microsoft Office products . What's more, if feedback is high voted there by other customers, it will be promising that Microsoft Product Team will take it into consideration when designing the next version in the future.

You can set the color of the title to be the same as the background color of the card
