I'm trying to create a report as following. I'm having trouble creating separate columns from the same fields with different date filters. Any ideas?

Create a matrix. Drag employee and level on rows and drill till the final level. The stepped Layout option is there in format option. make the off. Drag month to columns

You could use drill down feature of matrix visual. And I will suggest you to put Column "Level" first and then as a Drill down column put Name.
