How to create subheader for columns and allow measure to be pulled to column in matrix chart?

I have a dataset here

I need to create a matrix chart that looks like this

 Initially I tried it to do with measure, it works but I could not create the subheading like "A", "B", or "Total". I also tried to pull the measure of total time into the column of matrix, but it is not working as well.

Does anyone have an idea on how to do this?

This is my result. You can download the pbix file attached at bottom to see the changes I have made. It requires quite some work because the current table format cannot meet the need. I have added some tables and measures to help get the expected result. 

For the Total Time column, I currently return its result in seconds. To get it returned in "hh:mm:ss" format, we may need to create a calculation group. Let me know if you have any questions.

Thank you so much for your work and reply! Would you mind explain the dax for number of records? I don't quite get it. I ran the code, the total ans and total no ans column showed 0 for all rows.
