How to add a Microsoft form in front of powerbi Dashbaord?

Hi

I need help solving a problem for client.

I created a dashboard which is publicly published.

Now they want to add a form which will contain Name and email infront of that dashboard, so that they can know who entered the dashbaord.

When people will fill and submit the forms then he will see the dashboard reports. 

How can i do that? Is there any way to add forms in the dashabord?

Its like this ... when people will come through the dashboard link they will get Name and email form, they fill the form and submit then they will see the reports.

I think you will need to use something from Power Apps with Power BI
