Date Visual Randomly Reformatting

Hello,

I have a visual that records my organizations reviews, into good and bad, and then the net number of good reviews over time and I want it to be set to a date hierarchy, with only the month being in the hierarchy, but it will randomly reformat from a hierarchy to just the regular date and I'm not sure what is going on with it.

I am just not sure what is causing this switch, and this will need to get fixed somehow, as the viewers need to be able to see it in a hierarchy format, as the plain date format is too difficult to interpret. Thank you in advance!

In the data view  create the hierarchy:

In the report view, ensure the hierarchy created is on the axis as shown in the picture below:

Yes I know, I currently have the date as a hierarchy, my issue is that it will randomly switch between being a hierarchy, and being a regular date. 

This is my visual when it is correct, with the hierarchy, note that only the month is present, which is what I want it to be:

This is my visual after it will go through a random change and switch to a non-hierarchy date field:

There is nothing that I change in the background that causes this, I will just have a report viewer reach out to me that the visual has changed and I have to manually go in and change it back to the hierarchy and republish my report, which is frustrating to both the viewers and myself. The only possible thing that I can think of is that I changed the DAX code to my date table one time around a month ago to allow for further backtracking of the data, but I don't believe that this would cause ongoing problems now. My DAX Code that creates my date table is:

Dates = CALENDAR( DATE( YEAR( TODAY() )-1, 1, 1 ), DATE( YEAR( TODAY() ), 11, 30 ) )
