Is it possible to add comments to an individual data point or individual visual in Power BI Desktop?

It's not natively available within Power BIfor data points, but there is a general comment feature for a dashboard or report . But you can add comments in an Excel or SharePoint List and then show these comments in a Tooltip when you hover over the data. If you have the licence, you could build a Power App to collect the comments and even embed the Power App in the PBI Report
But the key here is structuring the Excel/SharePoint List correctly so that the comments show on the correct data.
You will need to think about the following possible dimensions, change to whatever is in your dataset
Load the data into Power BI, create relationships with your dimenesion tables. Create a tooltip page for the Comments

The mechanism to do this right now is via the canvas apps integration, if you want it to be available on the report right away you need to use something that supports direct query. There's tutorials in YouTube for it if you search for "Power BI writeback".
