Combine 3 date columns into 1 and perform a count

Hi,

I have a table with three date columns that represent the date a patient consented to a study, if appliable was 'randomised' into the study, and if applicable withdrew from the study. I have created columns to show counts for enrolled, randomised and exits by inserting a '1' if there was a date recorded, and a "0" if there was no date.

What I want is to convert this into 4 columns - there would be one date column which would range from the earliest 'Date of Informed Consent' to the latest of either 'Randomisation Date' or 'Date of study completion/withdrawal'. I'd like to show this by Month and Year only (per the data above). 

 

Next I want to count the number that either enrolled, randomised or exited for that Month and Year. 

 

Please help!
