Category with highest volume for individual

Hi, looking for some help please. I have a set of data that has individuals who have submitted forms for review. Some of the forms have errors, all of which are categorized. What I'm looking to create is a table with the individual's name and the category of error with the highest volume. A sample of what my data looks like is on the left and what I'm looking for is on the right.

@parry2k this is great, thank you so much! The first video got me 90% of the way there, and I went to the video before it (Let's explore INDEX function in Power BI and learn how it is different from other WINDOW functions) to get the final piece. This was great. Really appreciate your help.
