After create a form 1 in PQ how to add one more row in form 1 ?

I was told about below step to create Form 1.

I have created 2 row , may i know how to add the 3th row customer name ?

Hope some one can advise me.

Can you share sample data and sample output in table format? Or a sample pbix after removing sensitive data.
