As per Microsoft documentation, Fabric supports R custom (.tar.gz) library.

I want to install the library "janitor" at the environment level (scenario 2 in the documentation above). However, it always fails to publish.
I downloaded the package from the CRAN, uploaded it to Custom Libraries, and published it. It always fails to install the library.

Does anyone know if we can install R custom (.tar.gz) library at the environment level?
I know there is an option for inline installation. Since the Microsoft documentation shows it supports the R custom (.tar.gz) library, I am wondering why it always fail for me?

Have you tried to attach the other type of R custom packages or different version of janitor packages? AFAIK, normally the cloud service environment support the most stable version of packages instead of the latest version.
BTW, if the install to the environment failed, have you tried to use notebook inline install?
