How can I show the difference from the average # of clinics and # of clinics in percentage by writing DAX measure as you see in the right table?

Thank you very much for your quick reply. I divided # of clinics for each FSR(service area) by the average of all FSR Clinics, Finally subtracted 1 ### C3/avg(c$3:C$10)-1###.

It is not exactly clear how you obtain the percentages in the second table we see in your post. You should consider giving that information, too.
However, you might try to make a measure using the following DAX code, and see how it works out:
Delta Average Clinics = DIVIDE('Table'[#Clinics]) - 'Table'[#YourData], 'Table'[#YourData])
