Calculated column return Non-Blank Columns name and Values From Multiple Columns With Delimiters

I have following table:

and trying to create a calculated columns that returns comma separated Non-blank Column names along with their value. (goal is geneate the orange column values)

I used following and could only return the values not the column name

 

Calculated Col =
VAR __Table = {[A)],[B],[C)],[D] }
RETURN
CONCATENATEX(
FILTER(__Table,[Value]<>0),
[Value],
","
)

Column = CONCATENATEX( filter(union(row("Name","A","Value",[A]),row("Name","B","Value",[B]),row("Name","C","Value",[C]),row("Name","D","Value",[D])), not(ISBLANK([Value]))  ), [Name] & " : " & [Value] & " , ")
