COUNT the MAXIMUM

PLEASE HELP!!

... with a formula that I´m having a hard time solving.

I have three measurements: "Date", "OrderID" and "Medical History".

I need to COUNT the MAXIMUM "OrderID" (Different Records), among "Clinical History", of each "Date".

I mean, among several numbers of "OrderID", if "Medical Record"number is repeated by day ("Date"), COUNT the MAXIMUM number of "OrderID".
