Assign a score depending on measure value

I am trying to assign a score to new measure:

0  = 100/100
1  = 75/100
2  = 50/100
3+ = 0/100

I have found partial solution however I not sure how to start formula. 

Column =

    'SNC'[Disruption per month] >= 100  && 'SNC'[Disruption per month] = 0,

    'SNC'[Disruption per month] >= 75 && 'SNC'[Disruption per month] = 1,

    'SNC'[Disruption per month] >= 50 && 'SNC'[Disruption per month] = 2,

    'SNC'[Disruption per month] >= 0 && 'SNC'[Disruption per month] >= 3,

Any help will be greatly appreciated.

As another community member said, you can combine them using "switch" , as the following writes , for your reference.

Measure= switch('SNC'[Disruption per month]>=100,"0",>=75,"1",>=50,"2",>=0,"3+")
