We are currently on Oct 2020 and having bugs while refreshing "
“COM error: mscorlib, An item with the same key has already been added”"
Based on recommendation to update to Dec 2020 update, I have the following questions.
Can we update the server version to Dec 2020 or should we update to Nov 2020 and then Dec 2020?
should we update the PowerBI Desktop for Report Server as well to Dec 2020?
Some of the recently updated reports dont seem to refresh and throws errors.
Any feedback on this will be greatly appreciated...

That's a pretty obvious problem: your refresh is trying to add a row with a value that matches an existing value but the column is marked as being unique, that would changes the exiting model relationship. Please check the datasource and pbi model see what value it complains about, and why it is adding a duplicate to the current model.
Regarding to version updates, you should always update both Report Server and Desktop for RS to the newest version to able to prevent potential errors caused by feature updates. Just in stall the newest version Dec 2020, no need update to Nov then Dec.

I think you have the wrong Michelle Chan. I am a Program Manager in Azure Deployments and not do work with PowerBI server updates.
