    Hi! 

    I've attached a demo file. Take a look and let me know if it works for you. 

    I'm slightly confused as to why patient "G" isn't meeting the 2nd condition on the 19th / 03:00.

thanks again for your help. Sorry to ask again, but is it possible to send out an alert / email based on the last data entry and met condition 1. 

Appreciate the help.

I don't know whether or not it's posible to place a conditional trigger ("Check if something has happened and if it has send this mail") 

But you should be able to create a new page, filter out the most recent entry with condition 1 met, then finally create a subscription that sends out an e-mail with a snapshot of that page with the desired interval.
