Visual Help Needed

I am trying to deelop a visual to align with the following format:

 

Current month = this month

Previous month = last month

Historical = the month preceding last month

Historical = the month prior to the previous historical month

 

Effectively, I will have a 4 month trend. The color of the preceding 'dots' in those fields represent the position of the data point in relation to the A, T, L thresholds scale. For example, for Metric Name 2, current and previous months exceed the L threshold of 7%, thus marked as red. The first historical month value of 3.06% falls between A and L, categorixing it as T or yellow. The final column is equal to A, or represented as green. The black dot in the A, T, L thresholds section represents the position of the current month's data point relative to to the percentages.  The dot must adjust to the correct position based on current month's results. The count of months exceeding the threshols will equal how many months resulteing in either red or yellow, but once marked green, the count resets to zero (I knowthis can be done using a measure)..

 

What type of chart would I use and how would I get the colored dots and bars to resemble the example above?
