Utilization Rate for a Billed Procedure

I am trying to determine the utilization rate of a set of patients that received a PVI procedure that also used a special IVUS diagnostic tool.  Based on the hospital bill, I was able to identify the charge codes for PVI procedure, and the charge codes for the use of the IVUS tool.  I created a "PROC_PVI" conditional column that delineates a 1 for the charge of the PVI procedure.  And another "PROC_IVUS" conditional column that deliniates a 1 for the IVUS diagnostic tool charge.  However, there could be 3 scenarios that occur.  

 

1. The patient receives the PVI procedure(s) without IVUS diagnostic tool

2. The doctor uses the IVUS tool (1 or more), and then determines that a PVI procedure is not needed

3. The doctor uses the IVUS tool (1 or more), and then performs the PVI procedure(s) (1 or more)

 

How can I best determine the utilization rate of the IVUS diagnostic tool for PVI patients? 

 

Note: we are only counting the utilization in the procedure room.  In other words, if the doctor uses the IVUS tool more than once in the Operating Room (OR), it is only counted once in the numerator.  Or if the doctor performs more than one PVI procedure in the Operating Room (OR), it is only counted once in the denominator.  

 

Note: we do not want to include patients that DID received the IVUS diagnostic tool and DID NOT receive the PVI procedure.  However, we would like to point out how often that scenario occured.  

 

Any insight would be EXTREMELY helpful!

A colleague was able to help me define the measure as: 

= Divide (calculate(Distinctcount(HAR),Proc_IVUS = 1), calculate(distinctcount(HAR), Proc_PVI =1))
