Relationship error

Dear sir, 

 

Actually I am try to fetch the data between the mentioned tables. 

 

Relationship details attached, 

 

I am unable to get filtered values in appointment sheet according to the dates and i am unable to create relationship between dates and appointment tables. 

 

Kindly help me to clear this issue.
