    Please try:

    average =
CALCULATE (
    COUNTROWS ( 'Table' ) / DISTINCTCOUNT ( 'Table'[Client] ),
    FILTER (
        'Table',
        YEAR ( [date of visit] ) = YEAR ( MAX ( 'Table'[date of visit] ) )
            && MONTH ( [date of visit] ) = MONTH ( MAX ( 'Table'[date of visit] ) )
    )
)
