Presenting diffrent rolling 12 month period figures in a table visual

The following measures show how many people left the business in the rolling 12 month period, however I need this to be presented in a table as below therefore need a way of combining the two Dax measures below into one measure if possible as I want to avoid 12 measures for each column in the table.

Would appreciate any help on this!!!

P1 Leavers Prev Rolling 12 Prev Month = 
CALCULATE(
    COUNTROWS(Leavers),
            FILTER(
                    ALL('Leavers'[TERMINATION DATE]),
