Use Lat-Lon to plot the geographic location and you can use an arbitrary field, with whatever information you want in it) as the value in the Location section of the visualization.
You cannot arbitrarily structure the tooltip, though. You have to work within the structure of the visualization.
If you need to, you could make your own custom map visualization that allows customizing the tooltip information.

That works great, now I just need to figure out how to populate the Latitute & Longitute fields in the CRM!
