How to customize a radar chart into this

I want to create a radar chart like this where the circle lines are the number of activities are there and the dots are the one that basically stands on how many activites are done. And this is to be noted by each person like this.

The one I have currently only has 5 gridlines. I want it to have 6 and each of the dot that moves should denote one activity done 

Please do help.

Change the axis min value, or add 1 to the measure.
