Distinct column name values from 3 columns and lookup for values

Hi All,

I have table1 which has 7 columns and I would like to only get distinct name columns and provide values as per Names. Please help.

Table1:

Date	Primary Audit Name	Time Spent by Primary Audit	Secondary Audit Name	Time Spent by Secondary Audit	Tertiary Audit Name	Time Spent by Tertiary Audit
1/31/2020	Name1	2	Name7	3	Name3	2

Created a new table, Renamed columns. Add data to matrix visual
