We had a user come in today and when she opened her Power BI file (using Power BI desktop), she got a message stating "There are pending changes in your queries that haven't been applied". I see no pending changes, and this user is not advanced enough to edit queries. After clicking the Apply changes button, all of the visuals are broke, displayig "Something's wrong with one or more filters. Upon further examination, all relationships are gone, and all calculated columns and mesures in every table are gone. Also, when you click on the relationship button, it's as if the relationship diagram has been reset, that is, the tables are laid out horizontally across the screen, size of the tables are the same, etc. It's almost as if someone completely reset the data model. Is there any way, accidentally or otherwise, that a person could have done this?

Please direct to Query Editor and check the Applied Steps list to see which step caused the error. You could cancel the step one by one from the last step untile the error disappear.
Also, please try to find the recovery pbix file under this path: C:\Users\<username>\AppData\Local\Microsoft\Power BI Desktop\AutoRecovery.
