How can this be achieved? Please help.
@b_minesh - You could 'Unpivot' in Power Query however, I don't think that would be a good long term solution. I think you might want to take a look at CONCATENATEX as a measure understanding that you may not get the exact output you would like (below there are two entries [submittedTo] ID = 1 && [questionText] = "a").

Thank you, Chris. That worked for me. So my data looks like this now -
However, I need to add a rating value column and a date column to this visualization. When I put the rating value column in the values section of fields, each column in the visualization gets divided into two. Example is in the image below
How can I get just one rating column? Please help.
Try the Table visual instead of the Matrix visual. Otherwise you can try a more complex CONCATENATEX ( ) like https://www.sqlbi.com/articles/using-concatenatex-in-measures/
