If you are referring to publishing from PowerBI Desktop to PowerBI Service (PowerBI.com), then your reports are by default only accessible by yourself.
If you are referring to publishing a Report in the Service "to web" to embed it or link to it, then it will be accessible by anyone who has the link and may be discovered by other users.
Published reports/dashboards/datasets will only be visible to you unless you choose to share it within others within your organisation.
As you are probably aware, you can now share outside your organisation. However, this is only in trial. Anyone with the link will be able to see your report.
