Hello,
Creating an excel file connected to a datamodel will generate the usual blank PivotTable.
However, when refreshing the model (adding new columns for exemple), the excel file will be unable to integrate additional columns.
This behavior is similar to creating a model, publishing it, adding columns in the dataflow, and clicking refresh in the service. You first need to download all the data to a desktop model and republish it in order to see the new columns appears.
Is there something I am missing?

Where did you add the columns? In the query editor or directly added to the pivot table? If you mean the second one, this did not modify/update existing data model tables and structures.
In addition, Can you please share some more detailed information about these? They should help us clarify your scenario and test to troubleshoot.
