DAX formula to be able to reflect data on the table base on the matrix.

Hi guys,

I need your help on coming up with a formula to be able to reflect the "Actions to be done" based on the value on the table.

The matrix is as follows: 

1 Yellow = Meeting
2 Yellow consecutive months = Issue poor performance
3 Yellow  for consecutive months or 1 Red = NTE
2 Red for consecutive months = Conduct counseling
3 Red for consecutive month = Delisting

The table result should look a little like this.
