Conditional formatting index

I am creating an index to show what my conditional formatting values are so clients have a better sense of what they mean. 

I currently created a table with the 4 conditions and the order I want them to show. 

I threw this into a matrix and put the appropriate conditional formatting on the values. How do I get rid of the [Order] column in my matrix so only the [Index] shows? 

Or is there another way to put conditional formatting on just 1 row of values in a matrix?
