Implementing Conditional Formatting for Icon and Web URL

The documentation on conditional formatting mentions that conditional formatting can be applied to Color, Text, Icon and Web URL. I have successfully been able to implement conditional formatting for Color formatting. However, I have been unable to implement condtional formatting for Icons and Web URLs since I wasn't able to find any resources online. Any guidance on implementing the same would be much appreciated!

Thank you @lbendlin. However, my query is with respect to implementing Conditional Formatting for Custom Visuals inside the source code mentioned in this link.
