Trouble adding new data source to gateway

I'm trying to add a SQL Server data source to this gateway cluster, but it's not showing the other fields and the create button is grayed out. Any ideas what I'm doing wrong? Thanks!

Please follow the steps below to troubleshoot the issue.

1.Please check if the gateway is working: Is the gateway offline? Can the added data source use the gateway normally? Does the computer where the gateway is meet the installation conditions for the gateway? Please refer to the following link for the installation conditions of the gateway: Install an on-premises data gateway | Microsoft Docs

2.Are you the admin of the gateway? If not, ask the admin of the gateway to add you as an admin and try again.

3.If none of the above has problems, please uninstalling the gateway and installing the latest version of the gateway.

The gateway is online, and I've used it to connect to SQL Server using power automate, so I know it works. I've been given admin permissions for the gateway. 

This gateway has been set up by my company's IT department on a server, so I can't uninstall it and reinstall it. They're not sure why it's not working.

The only thing I can think of is that I don't have permission to use our 365 admin center. Would this affect it?

My first guess if it won't populate for you is that you have not been given permission to add Data Sources to the Gateway.  If you have permissions, what should happen is that after you select the cluster name, the DataSource Name and the Data Source Type options become available.  Therefore, my first step would be to confirm with the admin that you have permissions.
